When she decided she wanted to get plastic surgery this year, she could have turned to a number of renowned surgeons all over this country. Even her brother, Dr. Evan Garfein, is a well-known plastic surgeon in New York City. But rather than asking her brother to do the work — which she calls an “unwritten law in medicine” — she asked his partner at his New York practice,

Dr. Ashton, a double board-certified OB-GYN and obesity medicine physician, tells PEOPLE she’s always been “pro-plastic surgery,” so she suspected cosmetic procedures would be part of her life. It was after she left her role as Chief Medical Correspondent for ABC News andGood Morning Americaand started her own wellness experiment under her brand Agenda that she felt like now was the right time.
With her wellness journey, she says, “everything from the neck down” is “literally aging in reverse.”
“I am stronger and fit and actually have a better physique at 56 than I did at 36 or 46,” Dr. Ashton tells PEOPLE. “But the flip side of that is that as I’ve gotten so fit and so strong, I lost volume from my temples and my face, which is natural, and that happens with everyone, but it’s kind of an unexpected. So I just really felt like everything was aligning that it was the right time.”

On Oct. 7, Dr. Ashton got an upper blepharoplasty and autologous fat grafting after having a 3D scan done of her face to show potential results, a unique process that Dr. Tepper and Dr. Garfein offer.
“She was bothered by a bit of extra skin on her eyelids, and then she’s in such good shape and she’s so fit, which is wonderful, but sometimes that makes you lose some structure in the face and (have) loss of volume,” Dr. Tepper says of the procedure. “So with that, we decided to add a little bit of volume here in the cheeks, a little volume in the temples, and then we actually did some volume to the back of the hands.”
Dr. Tepper harvested the fat from Dr. Ashton’s sides and inner thighs, and she gushes over how much she loves what that did for her hands.

“My hands look like a 25-year-old!” she says while showing them off.
Both doctors note that the procedure and recovery went according to plan. Dr. Ashton notes that she had some bruising, but it was nothing that Dr. Tepper was concerned about.
“I had a decent amount of bruising under my eyes from the fat transfer, because again, that’s an area where there’s a lot of blood vessels, and it’s very delicate,” she says. “Nothing that couldn’t be covered up with makeup basically after one week.”

Dr. Tepper credits Dr. Ashton’s overall health for how well she handled the surgery and recovery. Several factors can affect how your body deals with surgery and the healing process, Dr. Tepper explains.
“Blood pressure is a big thing, and that’s stress-related, but obviously everyone has some inherent stressors and different levels of anxiety, but we really do encourage everything around you right before and right after to try to create a low stress scenario,” he says. “Whether it means making sure your (family) would help with your children or putting off other things that may be super demanding, because if you have those fluctuations in the blood pressure, that does contribute to a lot of the bruising and swelling. We sort of just say it’s really important that you relax after surgery.”
As a wellness expert, Dr. Ashton also created a nutrition plan that she followed before and after the surgery. It focused on decreasing inflammation, promoting wound healing and increasing collagen formation. It went so well for her — and Dr. Tepper agreed — that the two of them, plus Dr. Garfein are putting the whole practice into place for their future patients.

“I found it was so easy and so health-forward and intentional,” Dr. Ashton says of the new piece of Ajenda.
“I really believe in it, and I think she’s the perfect person to oversee it,” Dr. Tepper says of the new plan.
For Dr. Tepper, operating on someone who is not just a fellow doctor but also a friend meant that there was a slightly different approach. While he always takes the utmost care with his patients, he acknowledges that it’s certainly different operating on someone you know.
“It’s always a tough thing when it’s someone in your family or a close friend, and I’m very careful with that because it’s a different relationship when all of a sudden someone becomes doctor/patient,” he shares. “I also am careful that we’re going to be able to have a different dynamic to the relationship, which was super important to me. When I do have friends or family or people I know personally come, it’s almost like, look, we’re going to enter a new phase, and we have to be okay with that. You have to feel comfortable saying things you might not otherwise.”

As for what it’s like for a surgeon to operate on a surgeon? Dr. Ashton says she viewed it as a “positive” from her perspective because she knows the “beauty” of surgery and how a body heals.
For Dr. Tepper, he’s careful how he approaches the conversations with a fellow doctor.
“I think physicians, despite knowing a lot about general medicine, also appreciate that they’re a specialist for a reason, and so you don’t want to assume that a physician knows everything that you’re saying,” he says, adding that sometimes he’ll “over-explain” what he’s doing, just to make sure his bases are covered.

In the end, Dr. Ashton is “delighted, ecstatic, thrilled” with her results.
“My husband and my kids think I look 10 years younger, literally 10 years younger,” she says, adding that she’s also been forthcoming with her followers, whom she says expect “transparency” from her. She shared the full story in an Ajenda newsletter earlier this month and often discusses the ins and outs of her own health on her platforms.
“I feel like there’s so much fakeness in the world today that it’s so important to just be honest and authentic and transparent.”
